Sarah Zierfus, DULCE Case Manager
I've been with SAPCC since the fall of 2021!
I'm the DULCE Case Manager, embedded in the Mt. Ascutney Pediatric Clinic, supporting families with babies 1 to 12 months old. 👶 Right now, we're supporting 66 families, and that's just the little ones! I connect with families during well-child visits, home visits, follow-up calls, and I help with appointments, weight checks, and guidance around things like starting solids. 🍼🥄
I also support families with postpartum care, mental health, food, housing, transportation, finances, and more, connecting them to resources like WIC, legal aid, and The Women's Resource Center. 💪✨ Plus, I distribute essential items throughout the first year to help families thrive!
What I love most? ❤️ Being part of the DULCE program lets me combine case management with the medical side I love. Seeing families embrace the program and trusting me during some of the most vulnerable moments of their lives is incredibly rewarding.
